Exosomes

Exosomes are small extracellular vesicles, typically ranging in size from 30 to 150 nanometres, that play a crucial role in cell communication. They are released by various cell types, including skin and hair follicle cells, and carry proteins, lipids, mRNA, and microRNAs that influence the behaviour of other cells. Exosomes are an exciting and innovative therapeutic tool that can enhance the rejuvenation of skin and stimulate hair growth.

Skin Regeneration and Rejuvenation

Exosomes have gained significant attention in aesthetic dermatology for their potential to rejuvenate skin. They can be rich in growth factors, cytokines, and other bioactive molecules that promote cell regeneration, collagen synthesis, and tissue repair. They can be used in various aesthetic treatments to:

Stimulate Collagen Production: Exosomes encourage fibroblasts in the dermis to produce collagen, elastin, and hyaluronic acid, improving skin elasticity, texture, and overall appearance. This is particularly beneficial in treating fine lines, wrinkles, and sagging skin.

Promote Wound Healing: Exosomes can accelerate tissue repair, making them ideal for post-procedure recovery. Whether used after microneedling, laser treatments, or chemical peels, they can support faster healing, reduce inflammation, and improve overall skin texture.

Reduce Inflammation: Chronic inflammation is a major contributor to skin aging and conditions like acne and rosacea. Exosomes have anti-inflammatory properties that can help calm the skin and prevent further damage from environmental stressors.

Enhance Skin Hydration: By promoting the synthesis of hyaluronic acid and other skin moisturizers, exosomes can help improve skin hydration and create a plumper, more youthful appearance.

Hair Restoration

Exosomes are also gaining recognition for their ability to support hair restoration and regeneration. In aesthetic medicine, we commonly use exosome therapy for hair loss, particularly in conditions like androgenic alopecia (male and female pattern baldness) and telogen effluvium. Here’s how they work:

Stimulate Hair Follicle Growth: Exosomes from hair follicle stem cells contain growth factors and signalling molecules that can reactivate dormant hair follicles and stimulate the anagen (growth) phase of the hair cycle.

Increase Hair Density and Thickness: By encouraging the proliferation of dermal papilla cells and other hair follicle components, exosomes can increase the density of hair and improve the thickness of individual strands.

Reduce Hair Loss: The anti-inflammatory properties of exosomes can help reduce the shedding of hair by calming scalp inflammation and promoting a healthier scalp environment.

Improve Scalp Health: Exosomes also support the overall health of the scalp by enhancing the microenvironment where hair follicles exist, which can help in treating conditions like dandruff or seborrheic dermatitis.

Clinical Applications

In clinical practice, exosomes can be delivered through various methods, including:

  • Microneedling: Exosomes can be applied topically following microneedling to enhance the absorption and effectiveness of the exosome treatment.
  • Injectable Treatments: Exosomes can be injected directly into the dermis for skin rejuvenation or into the scalp for hair restoration.
  • CoolTarget: when combined with TargetCool, the results are enhanced through deeper absorption and reduced inflammation

Exosome therapy is generally well-tolerated, with minimal risk of adverse reactions since exosomes are naturally occuring, biocompatible and free from immunogenicity. Exosome treatment is often used in conjunction with other aesthetic treatments to maximise results.
